What Is History Summary?

THISChart History Summary is a single pane summary view of an individual patient's allergy, medication, procedure, problem, family and social history. This gives the clinician a rapid yet thorough overview of these critical data elements at the point of patient care, without multiple mouse clicks. Our History Summary offers providers the ability to drag and drop various history modules into their preferred sequence and the ability to customize the presentation of patient history information for personal workflow preference in one single view.

Allergy History

THISChart™ Allergy History allows review and editing of current allergies as well as the addition of new allergy information from one screen. It records the substance, type of allergy, specific reaction and source of the allergy information. Specific medications (lisinopril) or a drug class (ACE inhibitors) can be added. When a clinician orders a medication, patient allergy data is accessed by the THISChart™ CPOE module and an alert fires if an allergy is present.

Medication History

Like other THISChart™ modules, our design aim was to reduce the time required to accomplish each task. From one screen, a clinician can view current or past medications as well as prescribe, discontinue or reconcile medications. THISChart™ Medication History utilizes the Medi-Span® database, one of the most trusted and widely used drug databases in the world today.

Procedure History

Patient procedures such as surgeries, cardiac catheterizations, etc., are readily available to view. Procedure documentation is standardized by using CPT nomenclature and may be added by name, partial name or CPT code and sorted by multiple variables including provider, date and location. Details can be added to any procedure on the fly and frequently used procedures can be saved in your "Quick List" to speed documentation.

Problem History

In one location, view, add and update all of your patient's medical problems and diagnoses. THISChart™ allows the clinician to differentiate a problem or symptom (upper abdominal pain) from a diagnosis (acute cholecystitis). A diagnosis can also be designated as a "working diagnosis" if desired. Problem and diagnosis status may be changed to "inactive," allowing the user to toggle between active and all diagnoses with a single click. Problems and diagnoses can be added easily by searching the embedded ICD-9/10 database or by choosing from your "Quick List" - a very useful feature found in many other THISChart™ modules. Details such as date of onset, date of resolution and comments can be added to any problem or diagnosis.

Social History

THISChart Social History module gives an overview of an individual patient's social history including habits, home environment, employment status, marital status, resuscitation preference and advance directive status. THISChart™ Social History module gives an overview of an individual patient's social history including habits, home environment, employment status, marital status, resuscitation preference and advance directive status. Essential elements of the social history can be easily reviewed, edited and added on the fly. The components of the social history are fully customizable to fit your practice needs.

Family History

With minimal effort, record, edit and review an individual patient's family history. The module comes preloaded with the most common medical conditions relevant to family history, however it is fully customizable to meet your individual practice needs. With a single mouse click, THISChart™ can record that you have reviewed family history for documentation purposes.
